§ 34-92. Filing and contents of application.

  • Application for a permit required by this division shall be filed with the city manager and shall set forth:

    (1)

    The name and address of the applicant.

    (2)

    The trade name under which the applicant does or proposes to do business.

    (3)

    Where the vehicles will be kept when not in use.

    (4)

    The number of vehicles the applicant desires to operate.

    (5)

    Whether the applicant has been convicted of a violation of any federal, state or municipal law.

    (6)

    An agreement or stipulation that the applicant will operate and continue to operate during the period of time the permit shall remain in effect in accordance with applicable laws and regulations, as the same may, from time to time, be amended.

    (7)

    Any other information required by the city manager.
