Upon presentation to and approval by the city manager of the certificate required
by section 21-192, the city manager shall issue a health card to the applicant, containing, among other
items, the name of the holder, the date of expiration and a statement that the holder
has complied with the provisions of this division. The city manager shall refuse to
issue such card, if the applicant has either failed to furnish the certificate required
herein or is not free from any contagious or communicable disease.
