§ 34-40. Filing and contents of application.

Application for a certificate required by this division shall be filed with the city manager and shall set forth:

(1)

The name and address of the applicant.

(2)

The trade name under which the applicant does or proposes to do business.

(3)

Schedules for operation.

(4)

The number of vehicles the applicant desires to operate, the class, seating capacity, design and color scheme of each vehicle and the lettering and marks to be used thereon.

(5)

Whether the applicant has been convicted of the violation of any state or municipal law.

(6)

An agreement or stipulation that the applicant will operate and continue to operate during the period of time the certificate shall remain in effect.

Any other information required by the city manager.
